Bristol Royal Hospital for Children are hosting a Virtual transition evening online on Teams for those who have children who are aged 15 with congenital heart disease who will be transferring to the Bristol Heart Institute for their ongoing care.
It will be an opportunity for young people and their families to meet the team in preparation for their transfer. The evening is open to all patients in the network.
